Two very different communities
Why US Families Arrange Caregiver Services in Sri Lanka.
America holds two distinct Sri Lankan communities, and they call us for different reasons. One is professional and dispersed, in tech, medicine and academia, spread from Boston to the Bay Area.
The other is dense and long-established, concentrated in New York, above all on Staten Island. Different arrival stories, different work, different neighborhoods.
What both share is the schedule problem. American work culture leaves little room for a 3am phone call, and your parents' waking hours fall almost entirely inside your night. Professional caregiver services put a reliable adult in the room during those hours.
- Elderly care at home arranged entirely from the United States
- Home nursing care for diabetes, cardiac and post-stroke conditions
- A written welfare update after every shift, timed to your morning
- One coordination team, so you are not managing this over WhatsApp voice notes

The coverage question, answered plainly
What Medicare and US Coverage Do Not Cover Abroad.
Medicare stops at the border
Medicare generally does not pay for healthcare received outside the United States. Decades of payroll contributions do not follow your parents to Colombo.
Medicaid is state-based
Administered state by state and intended for care delivered domestically. It has no mechanism for reimbursing home care provided in Sri Lanka.
Private from the start
For most families care is therefore paid privately. The offsetting fact is that Sri Lankan rates make daily, in-home support genuinely attainable.
Verify the specifics for your parents' situation with Medicare rather than relying on assumptions. Social Security is a separate matter with its own rules for beneficiaries abroad, and the Social Security Administration publishes guidance covering it.
Long-Term Care Insurance and Care in Sri Lanka.
Here is the one place American families sometimes find money that other countries cannot. A number of US long-term care policies include an international benefit.
Coverage varies widely. Some policies pay a reduced percentage of the daily benefit for care delivered outside the United States. Others exclude it entirely, and older policies differ from newer ones written by the same carrier.
Because Sri Lankan daily rates are a fraction of American home care costs, even a partial international benefit can go a long way. It is worth ten minutes with your policy schedule before you assume you are paying everything yourself.
- Check whether your policy has an international or overseas benefit
- Note the percentage paid and any elimination period
- Ask what documentation the carrier requires for claims
- Confirm whether an agency placement qualifies, or only licensed facilities

03
We match a caregiver
Matching runs on language, temperament, clinical need and location. You review the profile before anyone starts work.
04
Updates after every shift
Meals, medication, mobility and mood, written up and sent by WhatsApp. Waiting for you at breakfast.

Wrong fit? Replaced free
Unlimited replacements under the annual membership. Judging a match from half a world away is hard, so we removed the cap.
American families ask about accountability earlier and more directly than families anywhere else we operate. That instinct is a good one, and the answer is that caregivers are employed and payrolled through BetterHands rather than introduced and forgotten.
Parents resisting help is the other common obstacle. It is rarely about the care itself. Most often it is about not wanting to become a burden to a child who already left. Many families therefore start with companion care instead of clinical support, because company is far easier to accept than help.

The consequence is stark. Your parents wake, eat, take medication, see a doctor and go back to bed entirely inside your night. By the time you are free to call, they are asleep.
For that reason the welfare update matters more here than in any other country. It is written at the close of the Sri Lankan day, which lands in the small hours on the East Coast and late evening on the West. Either way it is there before your alarm.
Live conversations work best in your early morning or late evening. Our coordination team schedules calls into those windows deliberately, so speaking to a person does not cost you a vacation day.
Wherever you settled
Caregiver Services in Sri Lanka by US City.
The care happens in Sri Lanka. These are the American cities our families call from.
Staten Island deserves its own mention. The Tompkinsville area holds one of the largest concentrations of Sri Lankans anywhere outside the country, dense enough that the neighborhood is widely known as Little Sri Lanka.
That density changes how families find us. In Staten Island, word of mouth does the work, and a bad experience would be common knowledge within a week. In Dallas or the Bay Area, families are often the only Sri Lankan household they know, and everything starts from a search instead.
We treat both as normal. Whether you were referred by three neighbors or found this page at midnight, the assessment and the guarantee are identical.
Distance from Sri Lanka also varies more across America than anywhere else we work. No route from any US city reaches Colombo without at least one stop, and West Coast families routinely spend twenty-four hours or more in transit. An emergency flight is a serious undertaking, which is precisely why steady, documented daily care carries more weight here than a promise to come quickly.

What US Families Ask Before They Commit.
How much does home care in Sri Lanka cost?
A one-time annual membership covers coordination, payroll and unlimited caregiver replacements. Daily rates then apply by staff role, from caregiver through to registered nurse. Rates appear below, and the assessment is free and comes first.
Do I need to fly over to set this up?
No. Assessment, matching, approval and payment run remotely from the United States. Your parents meet the caregiver in person. You review the profile, approve the plan, and receive written updates after every shift.
Who is accountable if standards slip?
BetterHands is. Caregivers are employed and payrolled through us, not introduced and forgotten. One team owns the placement, there is a number to call, and the replacement guarantee has no cap.

No district surcharge applies. A placement two hours from the capital costs the same daily rate as one in Colombo 7. Families discharged from the National Hospital of Sri Lanka or private hospitals are transitioned home directly.

Questions from the US
Frequently Asked Questions from US Families.
Yes. BetterHands runs a free assessment with your parents in Sri Lanka, matches a caregiver, and sends a written welfare update after every shift. You approve the care plan and pay from the United States. Nobody needs to fly over to set the placement up.
No. Medicare generally does not cover healthcare received outside the United States, and Medicaid is administered state by state for care delivered domestically. Home care for a parent living in Sri Lanka is a private arrangement, regardless of your parents' immigration or residency history.
Possibly, and this is worth checking carefully. Some US long-term care policies include an international benefit, often paying a reduced percentage of the daily rate. Read your policy schedule or call the carrier directly. It is the one funding route that sometimes works overseas.
Sri Lanka runs roughly ten and a half hours ahead of Eastern Time and thirteen and a half ahead of Pacific. Your parents' entire day happens while you sleep. Caregivers write the update at the end of the Sri Lankan shift, so it is waiting when you wake.
In many cases yes, though rules depend on citizenship, benefit type, and country. The Social Security Administration publishes guidance for beneficiaries living abroad. Confirm your parents' specific position with SSA rather than assuming, as this is separate from care funding.
BetterHands places caregivers island-wide. There is no district travel surcharge, so a placement in Kandy, Galle or Jaffna costs the same daily rate as one in Colombo. Availability for a specific area is confirmed during the free assessment.
You get unlimited replacements at no additional cost under the annual membership. Judging a match from eight thousand miles and half a day away is genuinely hard. The guarantee therefore has no cap for the life of your membership.
BetterHands nurses are registered with Sri Lanka's Private Health Services Regulatory Council. Coverage, Social Security and insurance questions should be confirmed with Medicare, the Social Security Administration, or your carrier, as individual circumstances vary.
Her whole day happens while you are asleep.
That is the plain arithmetic for Sri Lankan families in America. You cannot supervise from thirteen hours away, and a weekly call only tells you what your parents choose to share. Caregiver services in Sri Lanka replace that gap with something written down.
A vetted caregiver in the house. Home nursing when the condition calls for it. A written update after every shift, waiting before your alarm. One team accountable, with unlimited replacements if the match is wrong.
The assessment costs nothing and commits you to nothing. Sometimes it tells families their parents need less than they feared. Either way you stop waking up to silence.
